Job summary

The Salvation Army is seeking a part-time, non-exempt Case Manager at its 360 Life Center in Illinois. You will perform outreach, intake, needs assessment, direct assistance, and discharge planning, developing case plans and following up with clients to ensure ongoing support.

You will coordinate with funding partners, maintain HMIS/SIMS data, and refer clients to community resources while upholding privacy and program standards.

Qualifications

  • Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience.
  • 3–5 years knowledge and work experience related to Social Services and Case Management preferred.
  • Familiarity with Continuum of Care and homeless service providers.

Responsibilities

PAY RATE: $20.67 - $24.80 Hourly

The Salvation Army North and Central Illinois Division provides these services to people in need.

JOB SUMMARY

Responsible for case management services including: outreach, completing intake materials, assessment of client needs, provision of direct assistance, case plan development and compliance, discharge planning, and follow up care. Position includes providing resource referrals, screening participants for eligibility with various funding partners, and administering assistance according to agency and funder standards.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Conduct client intake interviews to determine eligibility for programs or agency referrals.
  • Provide case management services to clients to include, but not limited to: reviewing intake material for accuracy, assessing client needs, provision of direct assistance, case plan development and compliance, act as liaison to community resources, provide referrals to appropriate agencies, arrange for client transportation.
  • Ensure that all clients have access to a case manager through appointments and walk-ins.
  • Establish first contact within appropriate time frame.
  • Meet with clients weekly or as otherwise determined.
  • Attempt to meet for discharge planning.
  • Document client information and progress through HMIS, SIMS by WellSky, or other required database.
  • Record, track and submit statistical information as scheduled to maintain compliance with DHS, HUD, United Way, FEMA and The Salvation Army.
  • Attend monthly Continuum of Care (COC) meetings as directed.
  • Determine client eligibility.
  • Act as an advocate for clients when needed.
  • Maintain all client records ethically and confidentially.
  • Strive to meet the outcomes of programs and grant projects. Position works alongside the following initiatives:
  • Homeless Prevention (IDHS-HP)
  • Attend conferences, seminars and staff meetings as directed.
  • In administering programs, uphold the policies, goals and mission statement of The Salvation Army.
  • Maintain working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available.
  • Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices.
  • Maintain compliance in The Salvation Army’s “Safe From Harm” Certification.
  • Other duties as assigned by 360 Life Center Director.
  • Successfully satisfy the training requirements associated to the position and as required by The Salvation Army.

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES

Education:

Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience may be considered in lieu of a degree.

Experience:

A minimum of three to five years knowledge and work experience related to Social Services and supporting others through Case Management is preferred. Familiarity with local resources, Continuum of Care and homeless service providers.

Skills/Abilities:

  • Must have an interest and ability to work with people of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
  • Ability to establish policies and procedures; holding others accountable to the same.
  • Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member.
  • Demonstrated awareness of budget resources and cost control, as well as planning and administering budgets.
  • Proficiency in computer software applications, including demonstrated experience in using email, MS Office applications, and case management databases.
  • Close attention to detail and a high level of confidentiality.
  • Ability to multi-task several projects concurrently.
  • This individual will only commit resources of The Salvation Army that have been allocated or approved.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position. The individual will keep the 360 Life Center Director informed of all critical issues relating to their area of responsibility.
PHYSICAL DEMANDS/WORK ENVIRONMENT
  • This position may be required to do light physical work.
  • To successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment including a PC and make a public presentation.
  • The work environment for this position includes an office environment with a low to moderate noise level.
  • This individual must be able to travel to meet the needs of the position. A valid MVR is required.
REPORTING RELATIONSHIPS
  • This position reports to the 360 Life Center Director.
  • In contacts related to this position’s duties, this individual will interact with a wide range of contacts both internal and external. In these contacts he/she will act as a representative of The Salvation Army and its mission.

An evaluation of how effective service is provided. Performance will also be determined by outcome information provided during weekly staff meetings and monthly reports.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.
