The Salvation Army is seeking a Part-Time Case Manager at the 360 Life Center to provide comprehensive case management services, including intake, assessment of needs, direct assistance, and discharge planning. You will coordinate resource referrals and conduct eligibility screening in alignment with agency and funder standards.
You will document progress in HMIS/SIMS, maintain confidentiality, and support grant outcomes while working with diverse community partners to serve clients in need.

JOB SUMMARY
Responsible for case management services including: outreach, completing intake materials, assessment of client needs, provision of direct assistance, case plan development and compliance, discharge planning, and follow up care. Position includes providing resource referrals, screening participants for eligibility with various funding partners, and administering assistance according to agency and funder standards.

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES
Education:
Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience may be considered in lieu of a degree.
Experience:
A minimum of three to five years knowledge and work experience related to Social Services and supporting others through Case Management is preferred. Familiarity with local resources, Continuum of Care and homeless service providers.
