Job summary

Brain Injury Solutions is seeking a dedicated, full-time Community-Based Case Manager to support survivors of acquired brain injury toward recovery and independence. This home-office role requires face-to-face visits in Blacksburg/Christiansburg, Virginia, with occasional travel and comprehensive training provided.

Responsibilities include intake & assessment, building therapeutic relationships, coordinating services, and advocating across systems while maintaining documentation.

Qualifications

  • Knowledge of brain injury and its impact on individuals and families.
  • Understanding of case management principles and independent living strategies.
  • Ability to support clients with activities of daily living and community integration.
  • Strong communication, organization, and time management skills.
  • Ability to work independently while managing a dynamic caseload.
  • Proficiency in basic computer systems and documentation tools.
  • CPR/First-Aid certification (or willingness to obtain at our expense).

  • Manage client-related resources within approved budgets.
  • Support efforts related to CARF accreditation.
  • Participate in outreach, education, and public awareness activities.
  • Build and maintain relationships with community partners.
  • Support referral development and service awareness efforts.

Community-Based Case Manager

Reports To: Programs Manager. FLSA Status: Non-Exempt. Employment Status: Full-Time.

Salary: Starting at $46,500 with the potential to increase to $48,000 after 90 days based on performance. Location: Montgomery County, Virginia.

Are you passionate about making a real difference in the lives of others? Brain Injury Solutions is seeking a dedicated, full-time Case Manager to support survivors of acquired brain injury in their journey toward recovery and independence. This community-based position involves working directly with individuals and families to identify personal goals and connect them with resources that empower long-term success. The Case Manager will participate in ongoing marketing and outreach activities to promote agency programs, strengthen partnerships with community providers, and generate appropriate client referrals. This is a home office position, but face-to-face visits with clients in the aforementioned location is required. Comprehensive training is provided to ensure your confidence and success in the role.

What You’ll Do:
Intake & Assessment:

What We’re Looking For:

Education & Requirements:
  • Bachelor’s degree in a related field (e.g., rehabilitation, social work, psychology, human services).
  • At least 1 year of case management or direct service experience (brain injury experience preferred but not required).
  • CBIS, QBISP, or similar certification (or ability to obtain within required timeframe at our expense).
  • Valid driver’s license and reliable personal vehicle.
  • Current CPR/First Aid certification (or ability to obtain at our expense).
  • Background free of disqualifying offenses related to abuse or neglect.
  • Must reside in either Blacksburg or Christiansburg, Virginia.
  • Must be available every month (typically every third Thursday) for an in-person staff meeting in Wytheville from 10am-2pm. This meeting is compensated as time worked with mileage paid.
  • Must be available for in-person onboarding the first week of hire at our main office in Roanoke. Hotel accommodations provided if needed.
Work Environment & Physical Requirements:
  • Primarily home office-based with frequent travel throughout the service area.
  • Regular in-person services provided in client homes and community settings.
  • Frequent driving required.
  • Ability to communicate effectively across in-person and virtual settings.
  • Ability to remain stationary or move as needed to perform job duties.
  • Occasional lifting up to 50 pounds, with or without assistance.
