Job summary

Plan A Health in Vermont seeks a compassionate Case Manager who will provide community-based, family-focused support to high-risk youth, coordinating services across medical, mental health, and social supports.

Under supervision, you will assess needs, develop safety plans, and connect clients with resources, while collaborating with schools, DCF, and community partners to promote well-being and reduce out-of-home placements.

Qualifications

• Bachelor’s degree in social work, counseling, or related field with experience working with youth and families.
• Valid driver’s license required; experience in trauma-informed care preferred.
