Job summary

Youth Opportunity is seeking a dedicated Case Manager to provide comprehensive case management services for youth in care, under clinical supervision. You will coordinate treatment activities, facilitate communication among youth, families, and external partners, and assist in planning for successful community reintegration.

Responsibilities include developing treatment plans, coordinating assessments, and supporting post-release data collection, with emphasis on collaboration and compliance

Qualifications

• Bachelor’s degree in counseling, social work, psychology, rehabilitation, or related human services field.

• Prepare families for visitation, home leave, aftercare, and group/family meetings.
• Connect youth and families to appropriate community resources.
• Participate in internal audits and reviews to ensure policy compliance.

Job Type: Full-Time | Monday–Friday

Pay: $44,000 - $46,500 per year based on experience

Schedule: Primarily weekdays with flexibility for program needs

About Youth Opportunity & Bartlett Primary Assessment Center

Youth Opportunity is a nationally recognized leader in residential and behavioral health treatment for at‑risk youth. Bartlett Primary Assessment Center in Bartlett, Tennessee is a short‑term, trauma‑informed assessment facility serving youth in need of stabilization and behavioral evaluation.

Youth typically remain in care for approximately 30days, during which they receive structured intake, education, multidisciplinary assessment, and life skills programming. Small group settings support emotional safety, initial stabilization, and planning for an appropriate subsequent placement.

About The Role

As a Case Manager, you will work under the clinical supervision of a licensed mental health professional to deliver case management services for youth in care. You will coordinate treatment activities, facilitate communication between stakeholders, and help youth and families prepare for successful reintegration into their communities.

Key Responsibilities

Qualifications

• Bachelor’s degree in counseling, social work, psychology, rehabilitation, or related human services field
• Valid driver’s license and ability to pass background checks
• Emotionally and physically able to engage in youth activities and program duties

Preferred

• Two years of experience working with youth with emotional or substance use challenges
• Experience in juvenile justice, Medicaid-funded environments, or group facilitation
• Familiarity with treatment planning, case documentation, and family engagement strategies
