Northshore Home Health Care in Atlanta is seeking a dedicated Registered Nurse for remote care coordination. You will conduct telephonic and virtual assessments, develop individualized care plans, and coordinate with physicians and interdisciplinary teams to support chronic disease management and transitions of care.

Responsibilities include documenting encounters in EHR systems, identifying gaps in care, educating patients and families, and guiding care teams to improve service delivery.

Coordinate high-quality, patient-centered care for a caseload of members using telephonic and virtual assessment tools. Develop individualized care plans, facilitate access to appropriate services, and collaborate with physicians, specialists, and interdisciplinary teams to support chronic disease management, preventive care, and transitions between care settings.
