Job summary

Georgia Department of Public Health’s Specialty Care Clinic in Athens, GA seeks a Case Manager to join an interdisciplinary team serving individuals living with HIV. You will provide culturally responsive, trauma-informed services and help coordinate whole-person care toward viral suppression and improved quality of life.

You will assess clients, develop service plans, advocate, connect to community resources, and work with medical staff to support treatment adherence.

Qualifications

  • Master’s degree in Social Work and eligibility for professional licensure.
  • Experience in case management and HIV care is a plus.

Responsibilities

  • Conduct psychosocial assessments and identify client needs, strengths, and barriers to care.
  • Verify and maintain eligibility for Ryan White services.
  • Develop and update individualized service plans using a strengths-based approach.
  • Advocate for clients and address barriers to access to care.
  • Connect clients with community resources and support services.
  • Collaborate with medical and interdisciplinary team to support treatment adherence.
  • Provide crisis intervention and coordinate referrals for follow-up care.
  • Assist clients with applications for medication assistance programs.
  • Maintain knowledge of evidence-based practices and apply them to care.
  • Maintain timely documentation of assessments, interventions, and services.
  • Adhere to NASW Code of Ethics and clinic policies.

The Specialty Care Clinic is seeking a compassionate and dedicated Case Manager to join our interdisciplinary team serving individuals living with HIV.

This position plays an important role in helping clients overcome barriers to care and achieve positive health outcomes. The Case Manager will provide culturally responsive, trauma-informed services, including psychosocial assessments, individualized service planning, crisis intervention, advocacy, and connections to community resources.

Working alongside medical and other supportive services staff, the Case Manager will help coordinate whole-person care with the goal of supporting clients in achieving and maintaining viral suppression and improving their overall quality of life.

Job Responsibilities
  • Conduct comprehensive psychosocial assessments and identify client needs, strengths, and barriers to care.
  • Verify and maintain client eligibility for Ryan White services.
  • Develop and update individualized service plans using a strengths-based approach and in accordance with Ryan White Case Management Standard Operating Procedures.
  • Advocate for clients and assist in addressing barriers that may interfere with access to healthcare and supportive services.
  • Connect clients with appropriate community resources and support services.
  • Collaborate with medical and interdisciplinary team members to develop interventions that support treatment adherence and improved health outcomes.
  • Provide crisis intervention and coordinate referrals for appropriate follow-up care.
  • Assist clients with applications for medication assistance programs.
  • Maintain knowledge of current evidence-based practices and incorporate appropriate interventions into client care.
  • Maintain timely, accurate, and thorough documentation of assessments, interventions, communications, and services provided.
  • Adhere to the NASW Code of Ethics, clinic protocols, and applicable program requirements.
Minimum Qualifications

Master’s degree in Social Work and Counseling from an accredited college or university and eligibility for professional licensure.
