Responsibilities

• Provide basic case management services to long-term mentally ill individuals.
• Assess needs, plan goals and objectives, link individuals to community resources, and monitor to ensure needs are met.
• Offer counseling to individuals, including supportive counseling, family assessments and basic crisis intervention.
