Job summary

Bridge Over Troubled Waters is seeking a Case Management Specialist in Boston, MA. You will provide housing access and life-skills support to YYA residents, complete housing applications, and document all interactions in Bridge's ETO system.

You will facilitate skills-based groups, develop service plans, and collaborate with staff to connect youth with resources. Flexibility for field work and out-of-hours duties is required.

Qualifications

  • Bachelor's degree required; Master's preferred.
  • 2–4 years of clinical/case management experience with youth/young adults.
  • Experience building professional relationships with residents, staff and supervisors.
  • Experience assisting housing applications and obtaining IDs and vital records.
  • Experience facilitating skills-based groups (financial literacy, budgeting, life skills).

Responsibilities

  • Meet with YYA to assist with IDs, documents, referrals.
  • Complete CHAMPS Housing applications with youth as needed.
  • Conduct Housing Navigator searches and applications with youth.
  • Create and update individual service plans with YYA.
  • Document all interactions in ETO, Bridge's records system.
  • Lead life skills groups on topics like budgeting, credit building and stress management.
  • Meet residents monthly for case management; follow Bridge policies.
  • Track residents' savings, investments and progress; assist in planning monthly meetings.
  • Follow all policies and participate in program administration.

Our mission is to transform the lives of youth through safe, supportive and encouraging relationships, counseling, housing, and skill-building, to guide them towards self-sufficiency and achieving their goals.

ABOUT THE ROLE

The Case Management Specialist will provide case management support to youth residing in our Liberty & Brunson Houses. The Case Management Specialist will have demonstrated the ability to complete housing applications and obtain vital records/resources with young adults. Additionally, this position will provide individual and group work with youth teaching life skills such as budgeting/financial management, employment readiness and stress management curriculums, etc.

RESPONSIBILTIES

  • Meet with YYA to provide assistance in getting IDs and vital documents, referrals and general resources.
  • Complete CHAMPS Housing applicationswith youthas needed.
  • Complete Housing Navigator searches and housing applicationswith youth as needed.
  • Complete service plans with YYA.
  • Document all youth interactions inETO, Bridge's electronic records system.
  • Provide life skills groups to youth utilizing Bridge programs including financial management, building credit, establishing routines, managing stress, etc.
  • Provide residents with advocacy, referrals, and supportivecounseling,asnecessary.
  • Meet with residents from Liberty & Brunson House monthly for individual case management.
  • Complete the Millionaires Club Protocol withprospectiveresidentsinterested in living at Liberty & Brunson House.
  • Track residents’ work, savings, and investments monthly.
  • Assist in planning monthly house meetings with Liberty & Brunson House Residents.
  • Follow all of Bridge's policies and procedures.

Administrative Management:

  • Maintain program statistics and client records.
  • Document all interactions with YYA in ETO
  • Create individual service plans and YYA progress towards meeting theirgoals.
  • Complete HUD assessments asrequired.
  • Other duties as assigned.

QUALIFICATIONS

  • Experience in developing and maintain professional working relationships with residents, team members, and supervisors.
  • Experience assisting individuals in obtaining housing, getting IDs and vital records.
  • Experiencefacilitatingskills-based groups (i.e.,financial management, budgeting, life skills,etc.)
  • Experience using Motivational Interviewing and/or Collaborative Problem Solving.
  • 2-4years of clinical/case management experience with youth and young adult populations preferred.
  • Experience working effectively with diverse populations and cultural groups.
  • Bachelors required. Master's degree in social work or related field preferred.

Duties will be performed both in an office environment and in thefield,where program residentsreside. Because program residents have varied schedules, case management may take place out of regular business hours; a flexible schedule and ability and willingness to work outside of regular business hours is essential.

SKILLS AND STRENGTHS

  • Comfort and confidence working in a fast-paced dynamic work environment.
  • Enthusiastic about working with homeless transitional youth population.
  • Excellent written and verbal communication skills.
  • Proficient in Microsoft programs (excel, word, etc.) and utilizing electronic record systems.

PAY AND BENEFITS

The salary range for this position is $60,000 to $65,000. Benefits include:

PHYSICAL REQUIREMENTS

While performing the duties of this job the employee is regularly required to sit, work at a computer, stand and walk, and must be able to lift /move weight up to 25 pounds. The employee also must be able to hear and speak sufficiently to manage phone calls and effectively communicate/converse with others. The employee must have visual acuity and cognitive capacity to read, understand, and draft reports, letters, etc. The noise level is quiet to moderate, however, the employee must be able to perform in various settings, including in public spaces and at public events/activities.
