Job summary

Wayne HealthCare in Greenville, OH is seeking a Part-Time RN Case Manager with a hospital background to enhance patient management, maximize satisfaction and promote cost effectiveness. Responsibilities

• Case Manager duties include assessing, planning, implementing, monitoring, and evaluating actions to meet the client’s health and discharge services needs.
• Coordinate and provide care that is safe, timely, effective, efficient, equitable and client centered.
• Arrange services necessary for a safe discharge i.e. home health, SNF, outpatient services.
• Handle case assignments, review cases daily and provide concurrent review; submit necessary cases for insurance authorization.
• Develop effective working relations with the medical team throughout the case management process.
• Interact with patients to track progress and ensure satisfaction.
• Record case information and complete all forms/documentation accurately.
• Promote quality and cost-effective interventions and outcomes.
• Ability to multi-task and solve problems with excellent written and verbal skills.
• Work effectively in a self-directed role and communicate with insurers for authorization.

This is a part-time day shift, 32 hours per week position for an RN Case Manager with hospital background. Weekend rotation is required for every 4 th Saturday.
