Job summary

TriHealth Clinton Regional Hospital in Wilmington, OH is seeking a Case Management RN to lead utilization review and discharge planning. You will collaborate with physicians, patients, and care teams to optimize outcomes while supporting the transition of care.

The role will transition to Epic live later in the year, with a mix of project work and caseload responsibilities. The ideal candidate has 2–3 years in nursing and in case management or OB nursing, is proficient in EPIC and Microsoft

Qualifications

Bachelor's Degree in Allied Health required; BSN within five years for Clinton Regional Hospital staff
2–3 years nursing experience required
2–3 years case management or OB nursing experience required
Proficient in Microsoft Office and navigation of EPIC
Registered Nurse (RN)
Other Certified Case Manager within 2 years required
BLS certification required

Responsibilities

Facilitate advanced management of care for identified patient population
Coordinate patient care services across the continuum with patient/family and providers
Identify trends in clinical outcomes to drive system and process changes
Provide direction and support to patients, families, and health care providers on ethical/legal concerns
Participate in clinical quality improvement activities and evidence-based practice
Collaborate with professionals and external agencies to enhance case management services
