Qualifications

• Current, full, active, and unrestricted Registered Nurse license in the United States.
• Graduate of a school of professional nursing approved by a recognized accrediting agency.
• State licensure and appropriate pathway for nursing practice per VA standards.

Responsibilities

• Coordinates care with multiple providers across all levels and sites of care.
• Addresses psychosocial, nursing and medical needs of patients and families/caregivers.
• Evaluates care and outcomes to ensure timely provision of services.
• Assists Veterans, family members, and caregivers with options and services to meet complex health care needs.
• Focuses on patient education, self-management, and customer satisfaction across the continuum of care.
