Job summary
The Iowa City VA Medical Center is seeking a Contract Nursing Home Case Management Registered Nurse (RN) to provide and oversee evidence-based, age-appropriate patient care for Veterans receiving contracted nursing home services. RN coordinates comprehensive care across inpatient and outpatient settings, ensuring effective education, self-management support, and discharge planning.
Qualifications include English proficiency, an approved nursing program, and a current RN license in the US.
Qualifications
- English language proficiency required.
- Graduate of a nursing program accredited by ACEN/CCNE.
- Current, full, active RN license in a US state or territory.
- Bridge program pathway candidates may pursue licensure during/after program.
- Foreign education requires degree equivalency validation.
- Candidates must meet VA qualification standards and grade determinations.
Responsibilities
- Provide and oversee evidence-based, age-appropriate patient care.
- Coordinate comprehensive care across inpatient and outpatient settings.
- Educate patients and support self-management and discharge planning.
- Lead and collaborate with interdisciplinary team to improve outcomes.
- Participate in quality improvement and customer service initiatives.
- Assist Veterans and families with accessing required services.
