Oak Valley Hospital District in California is seeking a dynamic Case Manager for our Nursing Administration department who will embrace our mission, vision, and values.

The Case Manager will plan, organize, direct, coordinate and evaluate identified managed care patients following established criteria to maintain high quality patient care and ensure the department meets regulatory requirements for DHS and The Joint Commission. Collaboration with other managers is essential.

Qualifications

• Graduate from an accredited school of Nursing.
• CA RN licensure required; BSN preferred.
• Minimum of three years clinical experience; prior Case Management experience preferred.
• Utilization review experience preferred.
• California Public Health Certification preferred.

The Case Manager is responsible for planning, organizing, directing, coordinating and evaluating identified managed care patients following pre-established criteria to maintain high quality patient care. The Case Manager assures that the department meets all regulatory requirements related to service area for DHS, The Joint Commission, and others as appropriate. Working collaboratively with other managers to meet goals and solve problems is a requirement of this position.

JOB REQUIREMENTS

• Graduate from accredited school of Nursing
• Bachelor’s in science, Nursing (BSN) preferred
• Minimum of three (3) years’ of Clinical experience in a healthcare setting Previous Case Management experience preferred
• Utilization review experience is preferred
• Maintains current California Registered Nurse Licensure or has graduated from an accredited RN program and works directly under the supervision of a California licensed RN
• Prefer California Public Health Certification
