Healthcare Talent is assisting our client in their search for a Medical Case Manager in the Covina, San Bernardino, Orange and Northridge areas. This is a Care Management position in an advanced specialty collaborative practice, responsible for providing ongoing case management services for members. The Case Manager provides intensive case management in a process that includes assessment, planning, facilitation, implementation, coordination, monitoring and evaluation of the member’s needs. This position will be in the office one day a week; the remainder of the week will be spent in clinics with patients.

Job Description
- Performs comprehensive, disease specific, clinical assessment of all identified cases which includes but is not limited to:
- Assessment of member’s physical, functional, social and psychological status.
- Assessment of member’s cultural and linguistic needs.
- Assessment of caregiver resources and available benefits.
- Development and implementation of a member specific care plan which includes problems, interventions and goals.
- Schedule follow-up to assess progress towards goals and identify barriers to meeting goals.
- Communication with member’s physicians, specialists, community agencies and vendors to ensure coordination of services.
- Follows protocol for documenting all case interventions.
- Prepare and maintain appropriate documentation of patient care and progress within the care plan.
- Act as an advocate in the client’s best interest for necessary funding, treatment alternatives, timelines and coordination of care and frequent evaluations of progress and goals.
- Develop policies, procedures, desktop procedures, assessments and referral templates to facilitate the implementation of and compliance with new case management programs or requirements.
- Develop processes for oversight and reporting in compliance with new case management programs and requirements.
- Other projects and duties as assigned.
Qualifications
- Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program with current California license.
- Two to five years clinical experience, managed care experience preferred.
- Knowledge of healthcare or managed care environments preferred.
- Utilize and access computer programs – both Microsoft Office applications and job-specific applications.
