The Case Manager will plan, organize, direct, coordinate and evaluate identified managed care patients following established criteria to maintain high quality patient care and ensure the department meets regulatory requirements for DHS and The Joint Commission. Collaboration with other managers is essential.

Qualifications

• Graduate from an accredited school of Nursing.
• CA RN licensure required; BSN preferred.
• Minimum of three years clinical experience; prior Case Management experience preferred.
• Utilization review experience preferred.
• California Public Health Certification preferred.

Responsibilities

• Plan, organize, direct, coordinate and evaluate identified managed care patients per established criteria.
• Ensure department meets regulatory requirements for DHS, The Joint Commission, and others.
• Collaborate with other managers to meet goals and solve problems.
