Job description

A leading national hospice organization has an immediate opening for a full‑time RN Case Manager to support patients in the Walterboro region. This is a vital field‑based role providing compassionate hospice care coordination, comfort measures, and education to patients and families in their homes or care facilities. Flexibility is required as coverage may adjust based on patient needs and census.

You will manage a dedicated hospice caseload while delivering high‑quality clinical care and guiding families through the hospice journey with empathy and professionalism.

Responsibilities

• Perform comprehensive patient assessments and develop individual plans of care with the interdisciplinary care team.
• Make referrals to other hospice care disciplines as appropriate and document rationale clearly when not applicable.
• Maintain timely, accurate, and compliant patient records.
• Monitor and document changes in symptoms and overall condition.
• Educate patients, families, and caregivers on disease processes and care techniques.
• Communicate consistently with the clinical manager and the care team.
• Participate in on‑call rotation as assigned.
• Perform additional duties as needed.

Required Qualifications

• Current, active, and unencumbered RN license in the state of practice.
• At least 1 year of nursing experience.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and required liability insurance.

Preferred Qualifications

• CHPN (Certified Hospice and Palliative Nurse) certification.
• Experience in Hospice, Home Health, or Oncology.
• A strong background from hospice or home health organizations is ideal.
