Position: Full time - RN Case Manager

Qualifications:

• Recent (within the last year) acute care Case Manager experience preferred; candidates with 2-3 years of ICU or strong Med-Surg experience may be considered.
• Strong critical thinking skills and understanding of patient medical needs.
• Certification in Case Management, Nursing, or Utilization Review preferred.
• Florida RN license or approved multistate RN license required (Compact license holders must obtain Florida license within 60 days of relocation).
• BLS-AHA Provider card required within 30 days of start.
• Minimum 3 years of RN experience in an acute care setting.
• Graduate of an accredited RN program; BSN preferred.

Role responsibilities:

The RN Case Manager facilitates interdisciplinary care planning, focusing on evaluating medical necessity, admission status, level of care, and discharge planning. Coordinates activities to promote quality outcomes and patient throughput, identifies barriers, and facilitates discharge plans.
