Job summary

Interim HealthCare RMC in Indianapolis is seeking an experienced Registered Nurse Case Manager (RNCM) to coordinate in-home patient care and advocate for health outcomes. This role emphasizes one-to-one patient interaction, care planning, and collaboration with clinicians to maintain safe, comfortable home environments.

You'll conduct comprehensive assessments, develop plans, educate families, and help reduce hospital readmissions through preventative care and continuity of care.

Qualifications

• As a Registered Nurse Case Manager, you will coordinate care and advocate for patients in their homes.
• RN license and CPR/BLS/First Aid certifications are required.
• Minimum 1 year of professional RN experience in home health or similar setting.
• OASIS experience is preferred; case management experience preferred.

Responsibilities

• Conduct in-person patient interviews and comprehensive nursing assessments.
• Develop and evaluate individualized care plans and coordinate care with HHAs, LPNs, RNs, and other caregivers.
• Educate patients and families on home health procedures, wound care, IV administration, and medication management.
• Help reduce hospital readmissions through preventative care and continuity of care.
• Communicate patient conditions and collaborate with physicians and other providers as needs change.

What You’ll Do as an RN Case Manager:

As a Registered Nurse Case Manager (RNCM), you will coordinate and oversee the care of patients in their homes while serving as an advocate for their health and well-being.

• Conduct in-person patient interviews and comprehensive nursing assessments
• Develop, implement, and continually evaluate individualized patient plans of care
• Oversee and coordinate care provided by HHAs, LPNs, RNs, and other caregivers
• Communicate patient conditions and collaborate with physicians and other healthcare providers as patient needs change
• Educate patients and families on home health care procedures, including wound care, IV administration, and medication management
• Help reduce hospital readmissions through preventative care and continuity of care
• Monitor patient progress and make appropriate updates to the plan of care

What You’ll Need:

• Current, unrestricted RN license in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to and from patient care sitesAt least 1 year of professional RN experience in home health or a similar setting
• Previous case management or utilization review experience preferred
• OASIS experience preferred
• Practical tracheostomy and/or ventilator experience preferred, but not required
