Job summary

Rite of Passage is seeking a Case Management Coordinator for Walton Academy for Growth and Change in DeFuniak Springs, FL. You will oversee case management training, audits, and relationships with outside agencies to ensure treatment continuity for youth in residential care.

The role reports to the Program Director and may supervise Case Managers or Transitional Services staff. A Bachelor’s degree is required, with Master’s preferred, plus multiple years in human services and juvenile case

Qualifications

• Bachelor’s degree in related field (Master’s degree preferred).
• Four years of experience in Human Services, prefer two years of Rite of Passage experience.
• Prior case management experience required including three years’ experience with juveniles.
• Supervisory experience preferred.
• Knowledge of Placing Agencies, licensing requirements and Rite of Passage policies and procedures.
• Ability to pass a criminal background check, drug screen, physical and TB test.
• Ability to effectively communicate, verbally and in writing.
• Must be able to maintain a high level of confidentiality.
• Able to work in excess of 40 hours per week as needed.
• Must be flexible and able to handle multiple priorities.

Day in the Life:
