122 Massena Hospital is seeking a Care Facilitator to coordinate patient care across Massena, Canton-Potsdam, and Gouverneur hospitals. The per diem role features 8-hour weekend shifts, supporting utilization management, discharge planning, and referral to higher levels of care.

As part of the Case Management team, you will facilitate level of care determinations, monitor resource use, and collaborate with payors and clinicians to optimize care and reimbursement.

Competenze

• Diploma or Associate's degree in nursing (required).
• 3 years acute hospital care experience (required).
• Active NYS RN license (required).

Mansioni

• Conducts clinical reviews using Interqual criteria to determine level of care and support reimbursement.
• Monitors resource use with continued stay reviews and coordinates with Physician Advisors for complex cases.
• Liaises with payors to communicate clinical information, support appeals, and optimize benefits.
• Collaborates with care teams to resolve barriers to care and uses data for process improvements.

Nursing diploma or Associate's degree
