Rochester Regional Health is seeking a Care Facilitator to support Case Management. This per diem role covers Massena area hospital facilities, with 8-hour shifts and weekend coverage.

You will assess patient needs, coordinate care, and liaise with payors to optimize reimbursement. The position requires an RN license in New York, an accredited nursing diploma or an Associate's degree with 3 years of acute hospital care, and may prefer a Bachelor's in a related field.

Qualifications

• Diploma from an accredited nursing school program is required.
• Associate's Degree with a minimum of 3 years acute hospital care experience is required.
• RN license in New York State is required.
• Bachelor's Degree is preferred.

Responsibilities

• Conducts clinical reviews using Interqual criteria to determine appropriate patient level of care, document medical necessity, and support reimbursement.
• Monitors resource utilization through continued stay reviews, manages length of stay, and coordinates with Physician Advisors for complex cases.
• Serves as a liaison between hospital and payors, ensuring accurate communication of clinical information, supporting appeals, and optimizing benefit use.
• Collaborates with care teams to resolve barriers to care, participates in daily huddles, and uses data to identify opportunities for process improvement.
