Overview

Case Management Assistant with Vibra Healthcare at San Joaquin Valley Rehabilitation Hospital in Fresno, CA. This position involves clerical and administrative support for case management, discharge planning, and utilization review. Compensation: $21.00–$22.97 per hour.

Responsibilities

• Track dates for concurrent reviews due to third‑party payors.
• Obtain and fax updated clinical information from the EMR/Chart or managed care forms to payors.
• Confirm fax receipt and follow up to verify concurrent review information.
• Obtain authorizations and document them accurately in the HMS financial system.
• Assist with issuing Medicare Important Message after training.
• Help with initial post‑discharge referrals and follow‑up calls to providers regarding referrals and acceptance.
• Arrange post‑acute care services such as home health, durable medical equipment, outpatient dialysis, outpatient infusion, wound care centers, and post‑discharge transportation.
• Prepare discharge packets for skilled nursing, subacute, and acute rehab facilities.
• Assist in arranging post‑discharge physician appointments with primary care and specialists.
• Conduct post‑discharge calls to patients and providers when additional follow‑up is required.

Required Skills

• High‑school diploma or equivalent; knowledge of medical terminology.
• Minimum 1 year of experience as an administrative assistant or higher role.
• Previous experience in a health‑care or health‑insurance setting.
• Significant computer and data gathering and processing experience.

Additional Qualifications / Skills

• One year of college preferred.
• Acute‑care hospital experience preferred.
• Experience working with third‑party payors preferred.
• Knowledge of reimbursement programs, case‑management, and discharge planning preferred.
• Knowledge of federal and state regulatory standards and policies and procedures.
• Ability to understand and meet deadlines.
• Strong organizational, prioritizing, and analytical skills.

Posted Total Compensation

The wage range for this role is $15.75 to $19.20 per hour. Compensation decisions are based on factors including skill set, experience, education, training, licensure, certifications, and business needs.
