Responsibilities

• Enter and maintain progress notes in computerized charts and conduct chart reviews.
• Coordinate new referrals and eligibility evaluations.
• Develop Individualized Service Plans addressing concerns, needs and priorities.

Position Specific Information

Current grant funding through June 30th, 2027.

Shelter Plus Care is a federally grantfunded permanent supportive housing program that links housing withsupportive services to moveindividuals, and adults with families,who are homeless, have a disability and a low-income, to permanenthousing. Support forparticipants combines a rental assistancesubsidy funded by the U.S. Department of Housing and Urban Development'sContinuum of Care Program, and supportive services administered andprovided by Mecklenburg County Community Support Services and othercommunity partners.

This position assists householdsenrolled in Shelter Plus Care in housing navigation which includesdetermining fair market rate, rent calculations, housing search, andsecuring lease agreements. It also includes landlord recruitmentand retainment, assisting in households in mediating landlord/tenantissues.

Full time onsite Monday thruFriday, 8 am to 5 pm. Schedule flexibility needed for specialprojects and/or service and department needs. Office is located at 3205 FreedomDrive, Charlotte, NC 28208.

Community Support Services positions: This position MAY perform key job functions at acommunity partner work site. Employment in this position is conditionalupon following all policies and procedures of the community partner hostagency and adhering to all requirements that the community partner agencyhas established to grant and maintain physical and/or virtualaccess to the work site. If access is revoked, employment will beterminated.

Position Summary

Responsible for providing professionallevel case management or case coordination services for variouspopulations. This position, under general supervision, will participatein treatment planning and the creation and implementation of aperson-centered plan.

Essential Functions

• Review cases and provide supporting documentation regarding clients receiving financial assistance
• Interview and screen potential clients; review mental health and/or substance abuse history; conduct case consultations with applicable individuals; coordinate discharge planning
• Evaluate consumers immediate circumstances, including environmental situations, availability of services/support and develop service plans
• Prepare a variety of reports and documents related to client services, progress, and/or other related consumer issues and ensure all documentation adheres to State policies and procedures
• Foster inter-agency collaborations and research additional services or programs available to clients
• Coordinate new referrals and eligibility evaluations
• Develop Individualized Service Plans to address concerns, needs and priorities
• Arrange, link and integrate services with individuals and/or families
• Maintain compliance with procedural safeguards including client confidentiality and parental rights
• Enter and maintain progress notes and clinical services in computerized chart and conduct chart reviews
• Monitor client(s) progress on a regular basis via home/community visits, or phone contacts
• Assist clients in resolving concerns related to community resources, issues involving treatment or rehabilitation, and various living problems in home, work, or social environments; provides support as consumer copes with negative consequences of specific mental health issues
• May be required to staff shelters in the event of a disaster

Minimum Qualifications

• Experience:Minimum of two years in an applied settingsuch as mental health, substance use, or other health conditions, or incommunity related service.
• Education:Bachelor’s degree in Social Work,Psychology, Counseling, Sociology or any related human services field.

Preferred Qualifications

Master’s degree in a human servicesrelated field preferred.

Current or previous experience withhomeless populations and Housing First model.

Familiarity with trauma informedcare and Harm Reduction model.

Some accounting background helpfuland experience with grants.
