Job summary

Hartford HealthCare in Bridgeport, CT is seeking a Case Coordinator - RN to assess and coordinate patient care across the continuum, with a focus on discharge planning and complex clinical needs.

Qualifications

• Completion of a formal nursing program.
• Associate’s degree hires must obtain a BSN within 6 years of hire.
• Three years of acute-care nursing experience.
• Case management, home care, and/or managed care experience preferred.
• RN license from CT Department of Public Health.
• BLS certification preferred.
• Case Management Certification preferred.

Responsibilities

• Assess patient population in collaboration with the Health Care Team for appropriate level of care.
• Develop and implement discharge planning in collaboration with the Health Care Team.
• Collaborate daily with direct care providers to assess in-hospital stays and facilitate post-hospitalization services.
• Model quality patient service by maintaining collaborative relationships with caregivers.
• Provide ongoing assessment and develop interventions and treatment strategies.
• Advocate for patients’ needs and system changes to improve access to care.
• Educate patients, families, and professionals on disease prevention and treatment adherence.
• Maintain timely documentation of case management services.
• Identify discharge delays and develop countermeasures for barriers to discharge.
• Collaborate with providers to ensure appropriate patient placement throughout hospitalization.

Requirements

Education:

• Completion of a formal Nursing program.
• Associate’s degree hires must obtain a Nursing Baccalaureate Degree (BSN) within 6 years of hire.

Experience:

• Three years of acute-care setting, diversified clinical nursing experience.
• Case management, home care, and/or managed care experience strongly considered.

Licenses & Certifications:

• Licensed Registered Nurse credentialed from the Connecticut Department of Public Health.
• Certified BLS Provider credentialed from the American Heart Association (AHA) or the American Red Cross preferred.
• Case Management Certification preferred.
