Children’s Hospital of Michigan is an international leader in pediatric and adolescent medicine. Our surgical services include general, thoracic, reconstructive, and cardiovascular surgeries. We utilize imaging technology specifically designed for children, providing advanced diagnostic services such as Positron Emission Tomography (PET) and MRI. The Children’s Hospital of Michigan Emergency Department is a verified Level 1 Pediatric Trauma Center and a dedicated pediatric burn center. Our team of experts in pediatric critical care, rehabilitation, neonatal, and perinatal medicine care for thousands of children annually across Children’s Hospital of Michigan, Children’s Hospital of Michigan - Troy, and six ambulatory sites.
Under limited supervision and according to established policies and procedures, the Case Coordinator coordinates the implementation of treatment plans identified by healthcare providers in an ambulatory/outpatient setting.
Responsibilities include assessing clients to facilitate appropriate access and level of care, triaging patients, providing education, managing consultations and referrals, participating in team conferences, discharge planning, and conducting telephone follow-ups with clients.
