Job summary

Pyramid Healthcare seeks a dedicated Case Coordinator to manage and coordinate care for clients and families in residential treatment settings. You will provide individual and family case management, facilitate group education, and collaborate with the clinical team on admission, aftercare planning, and referrals.

Shifts are Monday–Friday, with 8:00am–4:00pm, 9:00am–5:00pm, or 10:00am–6:00pm options. This role offers comprehensive benefits and opportunities for professional development.

Qualifications

• Experience coordinating care in addiction or mental health settings preferred.
• Strong communication with referrals and providers required.
• Familiarity with documentation in clinical software such as Carelogic.

Responsibilities

• Coordinate client engagement through appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
• Maintain professional communication with referral sources and outside providers, assess needs, and document contact from admission through the duration of treatment.
• Provide case coordination services, including arranging transportation and conducting individual case coordination sessions.
• Assist in developing aftercare plans, including referral and aftercare provider contact, and addressing outside provider needs.
• Support clinical services by assisting with groups and supporting clients weekly to enhance engagement in the program.
• Collaborate with the treatment team and maintain professional communication with counselors and team members.
• Document client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines.
• Collaborate with the client navigation team for aftercare needs and offer onsite support for EOS appointments.
• Participate in mandatory corporate training, program training, and professional development.
• Perform other duties as assigned.
