Qualifications

• High School Diploma or equivalent required.
• One (1) year of experience in a drug and alcohol or mental health setting preferred.

Responsibilities

• Coordinate client engagement through appointments and aftercare planning with the treatment team.
• Coordinate communication with referral sources and outside providers.
• Develop and implement individualized aftercare plans and referrals.
• Document client, family, referral source, and provider contacts in the electronic health record.
• Participate in interdisciplinary treatment team meetings and maintain collaborative communication.
