Job summary

Pyramid Healthcare in Duncansville, PA seeks a Case Coordinator for our inpatient team. You will support transitions of care, coordinate aftercare services, and maintain communication with referral sources to ensure continuity throughout treatment.

The role focuses on coordinating aftercare, discharge planning, and collaboration with clinical and operational teams to support patient care, with a Monday–Friday schedule and a $15/hour pay rate.

Qualifications

• High school diploma or equivalent required.
• Experience in behavioral health, case management, or healthcare coordination preferred.
• Strong organizational and time management skills.
• Effective communication and interpersonal abilities.

Responsibilities

We are currently seeking a Case Coordinator to join our inpatient team. In this role, you will play a vital part in supporting patients’ transitions of care by coordinating aftercare services, maintaining communication with referral sources, and ensuring continuity throughout the treatment process.

Key Responsibilities

• Coordinate aftercare planning to ensure patients have access to appropriate next levels of care
• Schedule appointments, meetings, and follow-up services for patients
• Arrange halfway house interviews and placements as part of discharge planning
• Communicate with referral sources, families, and external agencies regarding patient progress (as appropriate)
• Obtain and maintain necessary documentation, releases, and records
• Support patients in navigating resources to promote successful recovery outcomes
• Maintain accurate records and documentation related to case coordination activities
• Collaborate with clinical, medical, and operational teams to support patient care
• Perform other duties as assigned

Qualifications

• High school diploma or equivalent required
• Experience in behavioral health, case management, or healthcare coordination preferred
• Strong organizational and time management skills
• Effective communication and interpersonal abilities
• Ability to work in a structured, fast-paced inpatient environment
• Basic computer and documentation proficiency
