Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients with daily living and personal care in their homes under an established care plan.

The role focuses on comfort, safety, and independence. You will follow the care plan, perform baths, hygiene, mobility support, medication reminders, and simple therapeutic activities, while documenting observations and communicating with the supervising nurse to ensure high-quality home health care.

Qualifications

• Able to read, follow written instructions and document care given.
• Maintain CPR certification per state regulation.
• Complete Maxim's competency program.
• Pass Maxim screening examinations.
• Meet federal, state, and local requirements.

Responsibilities

• Assist patients with daily living activities per care plan.
• Provide baths, oral hygiene, dressing, toileting, feeding.
• Take and record vital signs as ordered.
• Ensure safe transfers and ambulation.
• Observe and report patient condition to nurse.
• Maintain patient safety and use equipment properly.
• Document care and communicate with client/family and office.
