Job summary

Assisted Home Health & Hospice in Mission Hills, CA is seeking caregivers for shift work in the San Fernando Valley. You will assist patients with daily living activities and help maintain independence in a compassionate home-care setting.

Qualifications

• Completion of an approved training program pursuant to Federal/State law.
• Current, active California CHHAcertificate with criminal background check.
• Current CPR, health clearance and completed skills assessment.

Responsibilities

• Assist patient with personal hygiene, elimination, nutritional support, ambulation and exercise according to the patient’s needs and as specified on the CHHA Assignment.
• Ensure quality and safe delivery of CHHA services per written plan of care.
• Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patient’s condition and/or needs.
