Job summary

Direction-Home-of-Eastern-Ohi is seeking a Caregiver & Service Coordination Specialist to support unpaid family caregivers and eligible individuals across four Ohio counties. Under supervision, you will assess needs, coordinate referrals, and promote health, wellness, independence, and community safety.

You will be the primary contact for caregiver and Title III inquiries, develop caregiver education, and monitor service effectiveness while maintaining program standards.

Qualifications

• Bachelor’s degree required.
• At least 3 years experience in health care or related field.
• Must have valid driver’s license and auto insurance.
• Excellent communication and community outreach skills.
• Data entry experience.
• Ability to travel within the four-county area and occasional travel to Columbus.
• Criminal background check and drug screen required.

Responsibilities

• Assist unpaid family caregivers and eligible individuals in accessing information, resources and supports.
• Coordinate referrals to service providers and follow up on services.
• Conduct telephone and in-person assessments to identify needs.
• Develop, coordinate and deliver caregiver education and wellness initiatives.
• Monitor service effectiveness and fidelity to program standards.
