Job summary

Direction Home Of Eastern Ohio is seeking a Caregiver & Service Coordination Specialist to join the Main Office in Youngstown, OH. Under supervision, you will help unpaid family caregivers and eligible individuals access information, resources, and supports to stay healthy and independent in the community.

You will conduct assessments, coordinate referrals, monitor services, and contribute to caregiver education and wellness programs, while upholding program standards and collaborating with

Qualifications

Bachelor’s degree and at least three years in health care or related field.
Strong community engagement, communication, organization, and resource-navigation skills.
Data entry skills required.
Valid driver’s license and proof of automobile insurance.
Criminal background check and drug screen required.

Responsibilities

Assist caregivers and eligible individuals in accessing information, resources, and supports.
Serve as primary contact for caregiver and Title III inquiries and supports coordination of services.
Coordinate referrals to service providers and monitor service effectiveness.
Develop, coordinate, and deliver caregiver education and wellness initiatives.
Maintain fidelity to established program standards.
