Job summary

Diversity Group is seeking a Registered Nurse to join the Caregiver Support Program and deliver evidence-based outpatient care to Veterans. You will coordinate care, educate patients and caregivers, and support transitions through the care continuum in collaboration with interdisciplinary teams.

Responsibilities include patient education, self-management support, caregiver consultation, and program-related activities to improve care quality and veteran outcomes.

Qualifications

• Graduate from an accredited nursing program and licensed as a registered nurse.
• Ability to coordinate outpatient care and educate patients and caregivers.
• Proficiency in English for written and spoken communication.

Responsibilities

• Provide competent outpatient care to assigned patients.
• Coordinate care focused on patient education and self-management after discharge.
• Educate caregivers and veterans, and collaborate with the healthcare team.
• Engage in community outreach, resource development and quality improvement.
