Introduction

Family Ties Home Care has a client in Simi Valley who needs night time help so his wife can get her sleep. Client is in his 70s with dementia and is a fall risk. He needs to be watched and assisted when he gets up at night to use the restroom.

Responsibilities

• Following a pre-set care plan
• Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
• Assist with medication reminders
• Monitor and report on client's well‑being
• Light housekeeping, laundry, & other household daily chores as needed
• Grocery shopping & errands
• Cooking/meal preparation
• Scheduling appointments, including doctor's
• Communication with Family Ties HC management & family members/loved‑ones regarding the client's daily status
• Daily notes
• Other duties assigned as needed

Qualifications
