What You Will Do:

You will help implement customized care plans that address the whole person—mind, body, and spirit. Treat your clients like family, going above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care to clients in their homes
• Follow personalized weekly schedules and care plans
• Perform daily tasks safely and professionally (ADLs)
• Provide companionship and social/emotional support
• Assist with personal care activities, including hygiene, mobility, incontinence care, light housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment at all times
• Provide transportation to appointments as needed
• Document task completion and report changes in client condition
