Job summary

Vitality Living is seeking a Caregiver/Personal Care Specialist for the Hearthwood at Athens Community. The role supports residents with daily living, emotional well-being, and care plan execution in a warm, inclusive setting.

Ideal candidates are at least 18, fluent in English, and prepared to meet state health and certification requirements. The team emphasizes individuality and belonging, with flexible part-time opportunities and a supportive workplace culture.

Qualifications

• Must be at least 18 years old.
• Ability to communicate effectively in English, both verbally and in writing.
• Maintain any required state certifications.
• Meet state health-related requirements.

Responsibilities

• Assist residents with activities of daily living and emotional well-being as outlined in care plans.
• Participate in developing and updating service plans, reporting resident needs to supervisors.
• Observe and report resident issues to LPN or Medication Tech/Aid.
• Attend onboarding, annual and in-service training, and team meetings.
• Perform other duties as assigned by Wellness Director.

Caregiver/Personal Care Specialist Responsibilities:

• Assist residents with performance of activities, simple treatments, and emotional health and/or problem behavioral issues as indicated in service plans
• Assist in development and revision of service plans by informing supervisor of current resident needs and preferences, as well as successful service approaches
• Observe and report resident problems to LPN (if on duty) or Medication Tech/Aid
• Attend and participate in onboarding, annual and in-service training and team member meetings, as requested
• Perform other duties as assigned by Wellness Director
