Job summary

WelbeHealth PACE is seeking a dedicated Personal Care Assistant to deliver safe, compassionate care at our day center, reporting to the Day Center Manager. You will work within an interdisciplinary team to support participants in daily activities and participate in care planning.

Responsibilities include assisting with escorting, grooming, bathing, dressing, meals, and safety monitoring, while collaborating across departments to provide full-service care.

Qualifications

• High school diploma or equivalent required.
• Certification of Home Health Aide (HHA) or Certified Nursing Assistant in California preferred.
• Minimum of one year of experience providing personal care to seniors.
• Must be willing and able to safely drive to escort locations.
• CA Food Handlers Certification within 30 days of hire.
• CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

• Provide hands-on personal care assistance with activities of daily living (ADLs) to WelbeHealth participants, including escorting, toileting, bathing, dressing, grooming, laundry, and lifting when transferring participants.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort when needed, assisting participants with boarding and exiting WelbeHealth transportation vehicles.
• Oversee activity on the day center floor to assure participant safety.
• Assist with meal service and feeding when needed.
• Maintain personal care supplies such as incontinence supplies and grooming items.

The Personal Care Assistant is a vital part of our team, providing aid to our participants as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Day Center Manager, the Personal Care Assistant will provide safe delivery of personal care services to participants at the WelbeHealth day center.

This role is different because Personal Care Assistants at WelbeHealth:

• Collaborate with different departments within our day centers, building strong connections not only with participants, but with team members as well
• Unlike in a traditional assisted living facility, our Personal Care Assistants have the opportunity to care for seniors in a lively day center where our participants receive full-service care ranging from medical appointments, to rehabilitation services, to playing games and participating in activities

On the day-to-day, you will:

Job requirements include:

• High school diploma or equivalent required
• Certification of Home Health Aide (HHA) or Certified Nursing Assistant in California (CNA) preferred
• Minimum of one (1) year of experience providing personal care to seniors
• Must be willing and have the ability to safely drive to and from participant escort assignment locations, including but not limited to medical and care appointments in the program service area.
• CA Food Handlers Certification within 30 days of hire
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Compensation consists of base salary plus bonus. WelbeHealth offers a competitive total rewards package that includes a 401(k) match, comprehensive healthcare coverage, and a broad range of additional benefits. Actual compensation will be determined based on experience and relevant qualifications.
