Job summary

WelbeHealth in California is seeking a Personal Care Assistant to provide hands-on care to participants in our day centers, supporting ADLs and participating in care planning as part of an interdisciplinary team.

You will measure vitals, assist with transportation, ensure participant safety on the floor, help with meals, and maintain supplies. This role requires compassionate care and adherence to health and safety policies.

Qualifications

• High school diploma or equivalent required.
• Certification of Home Health Aide (HHA) or Certified Nursing Assistant in California (CNA) preferred.
• Minimum of one year of experience providing personal care to seniors.
• Must be willing and able to drive to participant locations, including medical and care appointments.
• CA Food Handlers Certification within 30 days of hire.
• CPR and First Aid certification within 180 days of hire (certification upon hire preferred).

Responsibilities

• Provide hands-on personal care assistance with activities of daily living (ADLs) to participants, including escorting, toileting, bathing, dressing, grooming, laundry, and assisting with lifting when transferring participants.
• Measure and document participants' vital signs as instructed.
• Act as a medical escort when needed, assisting participants with boarding/exiting WelbeHealth transportation vehicles.
• Oversee activity on the day center floor to assure participant safety.
• Assist with meal service and feeding as needed, ensuring correct meals are provided.
• Maintain personal care supplies such as incontinence supplies and grooming items.

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model.

The Personal Care Assistant is a vital part of our team, providing aid to our participants as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Day Center Manager, the Personal Care Assistant will provide safe delivery of personal care services to participants at the WelbeHealth day center.

This role is different becausePersonal Care Assistantsat WelbeHealth:

• Collaborate with different departments within our day centers, building strong connections not only with participants, but with team members as well
• Unlike in a traditional assisted living facility, our Personal Care Assistants have the opportunity to care for seniors in a lively day center where our participants receive full-service care ranging from medical appointments, to rehabilitation services, to playing games and participating in activities

On the day-to-day, you will:

Job requirements include:

• High school diploma or equivalent required
• Certification of Home Health Aide (HHA) or Certified Nursing Assistant in California (CNA) preferred
• Minimum of one (1) year of experience providing personal care to seniors
• Must be willing and have the ability to safely drive to and from participant escort assignment locations, including but not limited to medical and care appointments in the program service area.
• CA Food Handlers Certification within 30 days of hire
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
