Ivy Park at San Jose, managed by Oakmont Management Group, is a premier senior living community in California seeking a Care Provider. The role offers both full-time and part-time shifts, with AM, PM, and NOC options to support residents' daily living and independence.

The ideal candidate will provide compassionate care, maintain safety, and collaborate across departments to enhance resident quality of life. Training is provided, and benefits are available to eligible staff.

Qualifications

• Experience preferred but not required; training provided.

Responsibilities

• Assist residents with Activities of Daily Living (ADL) as per care plan.
• Respond promptly to resident needs while respecting dignity and confidentiality.
• Report changes in resident condition to supervisor immediately.
• Maintain a safe and secure environment following safety standards.
• Support teamwork through cooperation with colleagues and other departments.

Shifts, Time, and Days: Various Days, Various Shifts (AM 6:00am-2:00pm, PM 2:00pm-10:00pm, or NOC 10:00pm-6:00am)

Pay Range: $19.00 - $20.00

The Care Provider is responsible for providing personal assistance and daily care and services related to the residents in order to enhance the residents’ independence and quality of life, in accordance with the company’s mission and values.

Responsibilities

• Assist residents in their Activities of Daily Living (ADL) as specified in the resident’s service and care plan. This includes eating, bathing, dressing, toileting, transferring (walking), and continence.
• Respond to residents' needs promptly while maintaining residents' self-respect, dignity, safety, and confidentiality.
• Communicate any observed or suspected resident change of condition to a supervisor immediately.
• Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
• Encourage teamwork through cooperative interactions with co-workers and other departments

Qualifications

• Experience preferred but not necessary; we will train the right candidate
• Must be at least 18 years of age
• High School Diploma or GED
• Excellent customer service skills
• Strong desire to work with the elderly and care for their needs
• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests
• For the health and safety of our team members and residents, Oakmont Management Group may require team members to vaccinate, participate in daily screening, surveillance testing, and to wear face coverings and other personal protective equipment (PPE) to prevent the spread of the COVID-19 or other communicable diseases, per regulatory guidelines.
