Job summary

Oakmont Management Group seeks a Care Provider for Ivy Park at Pleasanton. The PM shift runs 2pm to 10pm, with a pay range of $18.00–$19.00 per hour, and a benefits package including health, dental, vision, paid time off, and retirement plans.

This role focuses on assisting residents with daily living, preserving dignity, safety, and independence while coordinating with the care team to deliver compassionate, client-centered service in a premier senior living community.

Qualifications

• Must be at least 18 years old.
• High School Diploma or GED required.
• Experience preferred but training provided.
• Excellent customer service skills.
• Strong desire to work with the elderly and care for their needs.
• Able to communicate effectively with staff, physicians, residents and families.
• Criminal background check and health screening required.
• Vaccination/health protocols may be required per regulatory guidelines.

Responsibilities

• Assist residents with Activities of Daily Living per care plans.
• Respond promptly to residents' needs while maintaining dignity and safety.
• Report changes in resident condition to a supervisor immediately.
• Maintain a safe environment following established safety standards.

The Care Provider is responsible for providing personal assistance and daily care and services related to the residents in order to enhance the residents’ independence and quality of life, in accordance with the company’s mission and values.

Responsibilities:

• Assist residents in their Activities of Daily Living (ADL) as specified in the resident’s service and care plan. This includes eating, bathing, dressing, toileting, transferring (walking), and continence.
• Respond to residents' needs promptly while maintaining residents' self-respect, dignity, safety, and confidentiality.
• Communicate any observed or suspected resident change of condition to a supervisor immediately.
• Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
• Encourage teamwork through cooperative interactions with co-workers and other departments

Qualifications:

• Experience preferred but not necessary; we will train the right candidate
• Must be at least 18 years of age
• High School Diploma or GED
• Excellent customer service skills
• Strong desire to work with the elderly and care for their needs
• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests
• For the health and safety of our team members and residents, Oakmont Management Group may require team members to vaccinate, participate in daily screening, surveillance testing, and to wear face coverings and other personal protective equipment (PPE) to prevent the spread of the COVID-19 or other communicable diseases, per regulatory guidelines.
