Job summary

Clarity Care, Inc. is seeking an Activity Support Provider to assist with Day Services programs in Wisconsin. No prior experience or certifications are required. Starting pay goes up to $18/hr based on experience, with opportunities for growth and advancement.

You will support members in daily activities, help with personal needs, and create engaging activities aligned with individual goals and interests. A valid Wisconsin driver’s license and background checks are required.

Qualifications

• Must possess a high school diploma/equivalent or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license and meet agency driving standards; provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check and/or state background checks.

Responsibilities

• Support members in community and recreational activities.
• Assist members with physical, social, and personal needs.
• Guide members in achieving personal goals and decisions.
• Create engaging activities tailored to member interests.
• Prepare and serve meals following dietary needs and preferences.
