Job summary

Clarity Care, Inc. seeks an Activity Support Provider to assist with Day Services. You will support members in community activities, help with daily living needs, and guide goal achievement in a caring, team-based environment.

No prior experience or certifications are required; training is provided. Competitive wages are offered with benefits and opportunities for advancement in Wisconsin Rapids.

Qualifications

Must possess a high school diploma/equivalent and/or related work/life experience.
Must be at least 18 years of age.
Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Responsibilities

Support members in community and recreational activities.
Assist members with physical, social, and personal needs.
Guide members in achieving personal goals and decision-making.
Create fun and engaging activities tailored to member interests.
Prepare and serve meals following dietary needs and preferences.
