Qualifications

• Excellent communication skills required for resident and family interactions.
• Compassionate, patient demeanor with a person-centered approach.
• Experience working with older adults or dementia is preferred.
• Ability to recognize changes in resident condition and escalate appropriately.
• Commitment to confidentiality and HIPAA compliance.
• Strong customer service skills and the ability to support residents and families.
• Ability to work independently and as part of a team in a caregiving setting.
• Basic documentation skills for care records and incident reporting.

Responsibilities

• Assist residents with activities of daily living including bathing, dressing, grooming, toileting, oral hygiene, incontinence care, ambulation, feeding and hydration.
• Observe residents for changes in physical and mental status and report promptly to licensed staff.
• Utilize dementia care techniques to promote dignity and reduce anxiety; monitor for wandering or elopement.
• Maintain a safe environment by identifying hazards and following infection control practices.
• Complete required documentation accurately and timely; report incidents and changes immediately.
• Engage residents in activities to promote social interaction and quality of life.
• Assist with meals, hydration, and dining setup; transport residents as needed.
• Communicate effectively with all departments and participate in meetings and training.
