Qualifications

• Experience delivering in-home care across settings (home care, assisted living, hospice, or long-term care).
• Background supporting individuals with developmental disabilities, autism, or specialized needs.
• Knowledge of dementia and memory care best practices.
• Ability to safely assist with lifting and transfers.
• Strong communication and interpersonal skills.
• A reliable, compassionate, and patient-centered approach.

Responsibilities

• Assist with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and mobility.
• Provide medication reminders and support daily health routines.
• Deliver specialized care for clients with Alzheimer's, dementia, or memory-related conditions.
• Prepare nutritious meals based on dietary needs and preferences.
• Perform light housekeeping to maintain a safe, comfortable environment.
• Safely assist with transfers and mobility, including equipment like Hoyer lifts.
• Monitor and document client condition, behavior, and progress.
• Maintain strict confidentiality.
