Transitions At Home, is currently seeking to hire a PRN Caregiver/Companion in the Mount Horeb area.

What does a Transitions at Home Caregiver do?
This Caregiver performs services for the clients as necessary to maintain the personal care and comfort of the client while in their place of residence. This client requires safety assurance, medication reminders, light housekeeping, meal prep and companionship. This is performed in accordance with the plan of care under the direction and supervision of the NM Manager.
Hours:
- 3:30pm - 6:30pm
- Tuesday, Thursday, Friday with the potential for consistent hours 5 days a week.

Qualifications / Job Requirements:
- Must be a minimum of 17 years of age
- One year of Caregiving experience preferred
- A valid Wisconsin Driver's License and Car Insurance
