• Proof of eligibility to work in the United States is required.

Responsibilities

• Provide assistance with ADLs and IADLs for Clients in home setting.
• Maintain communication and professional relationship with Clients and Family Members.
• Execute duties as assigned on personal care service plan.
• Keep the Client's environment clean and organized.
• Remind Client to take self-administered medications.
• Report changes in the Client's condition.

Job Responsibilities

• Provide assistance with ADLs and IADLs for Clients in the home setting (Hygiene, Incontinence Care, Mobility/Transfer Support, Light Housekeeping, Meal Preparation)
• Establish and maintain effective communication and a professional relationship with Clients, Family Members, and Co-workers
• Perform duties as assigned on personal care service plan, documentation upon completion
• Help keep the Client's environment clean and organized
• Remind Client to take self-administered medications
• Report changes in the Client's condition to provide quality of care
