Assurance Care & Support Services Inc. seeks Home Health Aides to provide one-to-one home care, including assistance with ADLs, mobility, medication reminders, and companionship. You will document care activities and report changes to the supervising nurse.

This role requires compassion, reliable transportation, and the ability to follow individualized care plans. AHA/HHA certification or PCA training is preferred, with background checks and drug screening required.

Responsabilidades

• Provide one-to-one care, assist with ADLs (bathing, dressing, grooming, toileting) per care plan.
• Assist with mobility, transfers, and prescribed exercises.
• Administer medication reminders and ensure adherence to treatment plans.
• Perform light housekeeping (meal prep, laundry, safety).
• Monitor and report changes in client condition to supervising nurse.
• Foster a positive, supportive atmosphere and companionship.
• Accompany clients to appointments, shopping, and activities as needed.
• Document care activities and maintain client confidentiality.
• Collaborate with healthcare team for holistic care.

• 1. One to One Care, Provide assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting, based on the individualized care plan.
• 2. Assist with mobility, transfers, and exercises as prescribed by healthcare professionals.
• 3. Administer medication reminders and ensure adherence to prescribed treatment plans.
• 4. Perform light housekeeping tasks such as meal preparation, laundry, and maintaining a clean and safe environment.
• 5. Monitor and report changes in the client's physical and mental condition to the supervising nurse or healthcare professional.
• 6. Foster a positive and supportive atmosphere, promoting emotional well-being and companionship.
• 7. Accompany clients to medical appointments, shopping, and other activities as needed.
• 8. Document care activities accurately and maintain confidentiality of client information.
• 9. Collaborate with the healthcare team to ensure a holistic and coordinated approach to client care.
