Job summary

Assurance Care & Support Services Inc is seeking compassionate Home Health Aides to provide one-to-one care in clients' homes. You will assist with daily living activities, mobility, and medication reminders, while documenting care and ensuring safety.

This role emphasizes empathy, reliable transportation, and adherence to care plans. Responsibilities include companionship, appointment accompaniment, light housekeeping, and collaboration with the healthcare team to deliver holistic client care.

Qualifications

• Certification as a Home Health Aide (HHA) or completion of a PCA training program or willingness to obtain.
• Experience in home healthcare or related field preferred.
• Compassion, empathy and genuine interest in helping others.
• Strong communication and interpersonal skills.
• Ability to follow care plans and adapt to client needs.
• Reliable transportation and a valid driver's license, if required.
• Ability to pass a background check and drug screening.

Responsibilities

• Provide one-to-one care and assist with activities of daily living (ADLs) per care plan.
• Assist with mobility, transfers, and prescribed exercises.
• Administer medication reminders and ensure adherence to plans.
• Perform light housekeeping, meal prep, laundry, and safety maintenance.
• Monitor changes in client's condition and report to supervising nurse.
• Offer emotional support and companionship to promote well-being.
• Accompany clients to appointments, shopping, and activities as needed.
• Document care activities and maintain client confidentiality.
• Collaborate with healthcare team for coordinated client care.

• 2. Assist with mobility, transfers, and exercises as prescribed by healthcare professionals.
• 3. Administer medication reminders and ensure adherence to prescribed treatment plans.
• 4. Perform light housekeeping tasks such as meal preparation, laundry, and maintaining a clean and safe environment.
• 5. Monitor and report changes in the client's physical and mental condition to the supervising nurse or healthcare professional.
• 6. Foster a positive and supportive atmosphere, promoting emotional well-being and companionship.
• 7. Accompany clients to medical appointments, shopping, and other activities as needed.
• 8. Document care activities accurately and maintain confidentiality of client information.
• 9. Collaborate with the healthcare team to ensure a holistic and coordinated approach to client care.
