Job summary

CareLinx by Sharecare is a nationwide caregiver registry connecting experienced in-home caregivers with health plan members and private clients in your area.

As an in-home caregiver on a part-time/per-diem basis, you set your availability in the CareLinx app and get matched to clients who fit your schedule and preferences, with support from a dedicated CarePartner team.

Qualifications

• 6+ months caregiving experience or relevant education/certification.
• Access to a smartphone capable of running the CareLinx app.
• Ability to communicate in English (bilingual encouraged).
• Reliable transportation to client homes.
• Must pass a background check.

Responsibilities

• Assist clients with activities of daily living (ADLs) including companionship, meals, mobility, bathing, grooming, dressing, and non-clinical medication reminders.
• Complete and document care tasks in the CareLinx app during each shift.
• Monitor and report changes to the client’s condition to the CarePartners team.
• Submit timesheets on time through the CareLinx app.
• Maintain patient privacy and dignity at all times.

WHAT YOU'LL DO

• Assistclients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
• Complete and document all care tasks accurately in theCareLinxmobile appduringeach shift
• Monitor and report changes in a client’s condition or self-care abilities to theCareLinxclinical team
• Submit timesheets on time through theCareLinxapp
• Communicate with your CarePartnerregardingany case updates or client changes
• Ensure patient privacy, dignity, and well-being in a safe and secure environment
• Maintain client confidentiality at all times

Additional shift opportunities (based on client needs):

• Transportation and errand support
• Hospital discharge escort — safelyaccompanyingmembers home after discharge
• Telehealth navigation — helping members connect with virtual doctor visits

WHAT WE'RE LOOKING FOR

Required qualifications:

• Minimum 6 months ofcaregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)
• Accessto a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets)
• Ability to communicate in English (bilingual caregivers strongly encouraged to apply)
• Reliable transportation to and from client homes
• Must pass a background check

Program-specific requirements (may apply depending on assignment):

• Completion of program-specific training prior to accepting shifts
• Medical clearance such as TB test, physical exam, or drug screening
• Valid driver's license and MVR check if accepting driving shifts
• Additionaldocumentation may berequiredbased on state law or client contract
